Parent Release Forms
A Parent Release Form must be filled out annually by the parent or guardian of each child or teen and must be provided to the church before they may participate in church activities. It's primary purpose is to provide the church with medical and insurance information and permission to provide medical treatment for the child or teen if that should prove necessary and the parent or guardian is not available.
The Parent Permission
Slip is used for parents to give permission for their child to participate in a specific church activity. A
Parent Release Form must have been previously provided to the church.
